The purpose of this questionnaire is to assist you in properly classifying student positions as employment, unpaid internship/volunteer and/or scholarship/fellowship. If you are interested in classifying a student as an independent contractor, this is not the correct worksheet. This worksheet will determine when payments made to students that are intended to be scholarship/fellowship actually represent compensation for services. Furthermore, the worksheet identifies when unpaid interns or volunteers must be paid as employees, in order to satisfy the requirements of the Fair Labor Standards Act. This form should be completed by the administering department for each student participating in a program or project.
